It is said that in 1546 tears flowed from the image of the Madonna frescoed on a wall of the portico of Octavia on the occasion of a crime committed in front of it. To commemorate the miraculous event, the fresco was detached and transferred to the nearby church of Santissimo Salvatore de Caccabariis. A confraternity was then established under the name of S. Maria del Pianto, which began construction of a church dedicated to the Virgin Mary in 1612, according to a design by Niccolò Sebregondi (1580-1652), which remained unfinished.
The Greek-cross interior is marked by pilasters. At the beginning of the nave is the banner depicting the Miracle of the Weeping Image of the Madonna and the Virgin in Glory, by Lazzaro Baldi (1624-1703). Also by the same artist Madonna and Saints (right cross). On the high altar is the venerated fresco of the Madonna del Pianto (15th century).
